Avascular necrosis, also known as osteonecrosis, is a condition where bone tissue dies due to insufficient blood supply. This lack of blood flow can occur following a bone fracture or joint dislocation, which may damage nearby blood vessels. Long-term use of high-dose steroid medications and excessive alcohol consumption are the primary risk factors for developing this condition. In its early stages, small cracks form in the bone, which may progressively weaken and eventually collapse.

If triggered by an injury, pain and stiffness may appear suddenly. In other cases, symptoms develop gradually over time. The main symptoms of avascular necrosis are:

  • pain in the affected joint
  • stiffness and reduced range of motion

The goal of treatment is to prevent further bone damage and preserve joint function. The approach depends on the severity of the condition.

In early-stage avascular necrosis, Dr Yu may recommend:

  • Medications
    Nonsteroidal anti-inflammatory drugs (NSAIDs) to reduce pain and inflammation
  • Rest & Activity Modification
    Reducing weight-bearing activities and using crutches to minimise joint stress
  • Exercise Therapy
    Gentle exercises to maintain joint mobility and function
  • Electrical Stimulation
    Using electric currents to stimulate bone growth and repair damaged tissue

In more advanced cases, Dr Yu will generally recommend surgery to restore blood supply and joint function. Surgical options include:

  • Core Decompression
    Removing a portion of the bone’s inner layer to relieve pressure and encourage new blood vessel formation
  • Bone Grafting
    Transplanting healthy bone tissue from another part of the body to replace damaged bone
  • Osteotomy (Bone Reshaping)
    Restructuring the bone to redistribute weight and reduce stress on the affected area
  • Joint Replacement
    In severe cases where the bone has collapsed, a total joint replacement may be necessary.
